Service Request forms
A few things to note when uploading your forms:
- Please do not include banking information on the form.
- Sign with a written or digital signature.
- Use the “edit text and image” function when completing the PDF fillable form to avoid uploading a blank document (“fill and sign” does not always work).
- Your file name should be Student ID_Last Name, First Initial_Type of Form.
If you are a current student or alumni, request a formal name change on your student record by completing and uploading the Name Change form to Mosaic.
Request a new or reissued diploma using the the diploma request form.
Recent graduates who want to enrol in courses can submit a request for continuing status enrolment.
Graduating students, who in extenuating circumstances, need to be conferred ahead of their scheduled ceremony can submit a request for early conferral.
Students can request an enrolment verification letter for immigration, study permit, visa and admissions purposes. Please allow 10-15 days for processing. Submit an enrolment verification request.
A re-read appeal is when you question your grade or assessment and allege error in the academic assessment of your work on the part of an instructor or a committee. Please review the details regarding a re-read appeal on the McMaster Secretariat website before submitting. Download the Re-read Application (Form A) and submit it in Mosaic.
To update your citizenship status on file, please submit the citizenship status update form.
If you are not able to continue at the University and you wish to apply for reinstatement, you must submit a Reinstatement Request form in Mosaic by the appropriate deadline.
