Upload paper forms to MacDrive
MacDrive is McMaster University’s cloud server where you can share your files with us. Similar to DropBox, it allows you to securely submit forms, requests and supporting documentation for review and assessment.
A few things to note when uploading your forms:
- Please do not include banking information on the form.
- Sign with a written or digital signature.
- Use the “edit text and image” function when completing the PDF fillable form to avoid uploading a blank document (“fill and sign” does not always work).
- Upload only one file to the MacDrive folder.
- Your file name should be Student ID_Last Name, First Initial_Type of Form.
- Forms are not accepted by mail or fax.
If you are a recent grad (up to one calendar year after your convocation) and haven’t yet received your diploma, you can request the delivery of your original diploma by filling out the After Convocation Diploma Delivery form and upload it to MacDrive.

If you are requesting to reprint your diploma with a change to your name, please upload both the Name Change form and the diploma replacement form to the diploma replacement MacDrive folder.
Interested in taking university courses at McMaster, but not for credit? Apply to become a listener by uploading the Listener form to MacDrive.
Name change requests for current students and alumni
If you are a current student or alumni, request a formal name change on your student record by completing and uploading the Name Change form to MacDrive.
Name change requests for graduating students
If you are a graduating student, request a formal name change on your student record by completing and uploading the Name Change form to MacDrive.
If you are not able to continue at the University and you wish to apply for reinstatement, you must submit a Reinstatement Request form by the appropriate deadline.
A re-read appeal is when you question your grade or assessment and allege error in the academic assessment of your work on the part of an instructor or a committee. Please review the details regarding a re-read appeal on the McMaster Secretariat website before submitting. Download the Re-read Application (Form A) and submit it to our MacDrive folder.
