GRADUATION INFORMATION CARD

The deadline has passed to submit a Graduation Information Card. For more information, please call (905) 525-4600.

 

  • The Graduation Information Card will allow confirmation of the name which will appear on the diploma and is the method by which students apply for a minor.  
  • Before completing your Graduation Information Card you should first go to MUGSI and check under My Academics/Program & Course Summary to see how the University has your surname and given names recorded in our records. This is how your name will appear on your diploma. If you want to add a second or third given name or surname please visit the Office of the Registrar during regular business hours and present one of the following original documents - name change certificate, birth certificate, current passport, marriage certificate or current driver's license - in order to support your request. Requests must be completed and supporting documentation received no later than October 7, 2011 for the Fall ceremonies.
  • You may choose through completion of the Graduation Information Card to list only those given names you wish printed on your diploma. Please note at least one given name must appear on your diploma.

Please Note:

Should you require a name change or correction of the spelling of one of your names recorded in the University's records (i.e. marriage, legal name change, additional given names) you will need to visit the Office of the Registrar to complete a name change request form and present one of the following original documents - name change certificate, birth certificate, current passport, marriage certificate or current driver's license - in order to support your request.
